Retinal Nerve Fiber Layer Evaluation in Patients Suffering from Multiple Sclerosis Using Optical Coherence Tomography – Literature Review

Summary: Optical coherence tomography (OCT) is a technique which can be helpful to understand mechanisms of brain tissue injury in multiple sclerosis (MS), other optic neuropathies and neurological diseases. In our study, we evaluate the usefulness of optical coherence tomography in retinal nerve fiber layer (RNFL) thickness measurement in patients suffering from MS with or without optic neuritis. Multiple sclerosis is an inflammatory, demyelinating disease with axonal degeneration and commonly appears with retrobulbar optic nerve damage. Optical coherence tomography is a noninvasive technique that quantifies the thickness of the retinal nerve fiber layer within the area around the optic nerve disc. In a systematic optical coherence tomography examination of multiple sclerosis retinal nerve fiber layer thickness is reduced when compared to disease–free controls. These changes occur even in the absence of a history of demyelinating optic neuritis in patients suffering from multiple sclerosis. Presence of defect in retinal nerve fiber layer in patients suffering from multiple sclerosis without retrobulbar neuritis may suggest subclinical optic nerve damage. Multiple sclerosis treatment may be a protective factor in the retinal nerve fiber layer loss. Optical coherence tomography has many records which permit comparing retinal nerve fiber layer thickness among successive examinations in a period of time in the same patient. This technique may provide complementary informations to multifocal visual evoked potentials (mfVEP). This examination and scanning laser polarymetry (Gdx) show good agreement.
