The Usefulness of Assessing the Thickness of Retinal Ganglion Cell Complex in Optic Neuritis in Children – Preliminary Reports

Summary: Aim: The assessment of the usefulness of mean and minimum thickness of ganglion cells and inner plexiform layer measurements in the diagnosis of optic nerve inflammation in children.
Patients and Methods: We retrospectively analyzed 69 eyes: 33 eyes of 28 patients with first uni or bilateral optic nerve inflammation (study group), and 36 healthy eyes of 28 children (comparative group). The mean age of patients in both groups was 13.8 years. We evaluated patient’s distance best corrected visual acuity and corrected for near vision visual acuity at admission and discharge, visual field mean deviation, the result of pattern visual evoked potentials and mean and minimum ganglion cells layer + inner plexiform layer thickness. Patients with significant refraction error were excluded from both groups.
Results: The mean and minimum mean and minimum ganglion cells layer + inner plexiform layer thickness in the study group was respectively 82.0 µm and 79.0 µm, in the comparative group 84.5 µm and 80.5 µm. In the first group mean corrected for distance visual acuity at admission was 0.3, corrected for near vision visual acuity D 0.8 and at discharge 0.6 and D 0.6. In comparative group mean and corrected for near vision visual acuity were 1.0 and D 0.5 both at admission and discharge. The average visual field mean deviation was 9.7 dB in study group and 3.0 dB in comparative group. In study group in 9 cases optic neuritis was confirmed by visual evoked potentials, in 19 result was inconclusive and in 3 correct. We found no statistically significant difference between measurements of mean and minimum ganglion cells layer + inner plexiform layer thickness between groups. However, there was a strong positive correlation between the average and the minimum ganglion cells layer + inner plexiform layer thickness in study group. Only the difference in visual field mean deviation proved to be a statistically significant between study and comparative group.
Conclusion: Assessment of the mean and minimum ganglion cells layer + inner plexiform layer thickness may be useful examination in the diagnosis and prognosis of visual function in children with optic neuritis in future. Therefore, at the moment accurate clinical evaluation and traditional diagnostic methods seem to be the most useful diagnostic and prognostic tools in optic neuritis.
