Novel Diagnostic Methods in Dry Eye Disease

Summary: Recently, dry eye disease is one of the most frequently diagnosed ophthalmic conditions. According to the definition developed during International Dry Eye Workshop II 2017, dry eye disease is “a multifactorial disease of the ocular surface characterized by the loss of homeostasis of the tear film, and accompanied by ocular symptoms, in which tear film instability and hyperosmolarity, ocular surface inflammation and damage, and neurosensory abnormalities play etiological role”. Nowadays, several diagnostic tools are used to establish the diagnosis of dry eye disease: slit-lamp examination, tear break-up time (BUT), Schirmer test, fluorescein staining, rose bengal staining, lisamine green staining, and special questionnaires (Ocular Surface Disease Index, Symptom Assessment in Dry Eye, McMonnies questionnaire, Dry Eye Questionnaire). Nevertheless, none of the available diagnostic methods is considered a gold standard in the diagnosis of dry eye disease. According to the international literature, novel diagnostic methods include optical coherence tomography, tear film stability analysis, thermography, the evaluation of lipid layer of the tear film by interferometer, biomarkers (metalloproteinase 9, IL-1Ra, IL-8, IL-17a, IL-6, IL-23, TNF-alpha). The aim of the study was to present novel diagnostic methods in dry eye disease. Large clinical trials are needed to evaluate the potential value of novel tools in the diagnosis of dry eye disease.
