New Approach for Tear Film Quality Assessment in Contact Lens Wearers

Summary: Purpose:To expose the problem of the lack of lens material biocompatibility with tear film of an individual contact lens user. To provide a brief overview of the methods for non-invasive in vivo measurement of tear film dynamics on contact lenses and to summarise the results of the latest developments in this field. Material and Methods: Dynamic changes of tear film in interblink interval can be assessed with interferometry or videokeratoscopy. The results of such measurements can be quantified as a time series, from which one can determine parameters characterising the quality of tear film. Results: Non-invasive methods for measuring tear film have the ability to distinguish healthy subjects from those having dry eye syndrome, to discriminate contact lens materials in vivo, as well as to estimate the lens dewetting time, a phenomenon that has been observed in 80% of measurements of tear film dynamics on contact lenses. Conclusions: Dewetting is the main mechanism responsible for the sudden deterioration pre-lens tear film quality. The level of contact lens material biocompatibility with the tear film of an individual user decides about its dynamics. Not optimally selected contact lens material could result in poorer quality and stability of tear film than that observed in dry eye. More attention should be given to the assessment of tear film during contact lens fitting procedure to reduce the risk of discomfort that could lead in future to discontinuation of contact lens wear.
