Summary: Hyperglycemia plays an important role in diabetic retinopathy pathomechanism. It activates several damaging pathways: polyol pathway, AGEs (advanced glycation and products) formation, PKC (protein kinase C) and hexosamine pathway, which are responsible for pathological changes in microvasculature of the retina and neural and glial cells in the retina. An increased reactive oxygen species (ROS) level in the retina is characteristic to diabetes and oxidative stress can lead to cells apoptosis (especially in pericytes and endothelial cells). Alpha-lipoic acid inhibits pathological changes in diabetes concerning cells and blood vessels (diabetic neuropathy and angiopathy). Vitamin B1 augments some alpha-lipoic acid effects. Thus alpha-lipoic acid and vitamin B1 supplementation may contribute to reduction diabetic complications on both the level of individual organs and systemic in patients with diabetic.
