Application of Nonsteroidal Anti-inflammatory Drugs in Ophtalmology

Summary: Nonsteroidal anti-inflammatory drugs is a group of medicines of wide use mainly in a form of systemic preparations administered generally, intravenously or orally. Local ophthalmic formulations are used in selected cases. Similarly to steroids they inhibit arachidonic acid cascade, limiting inflammatory mediators , i.e. prostaglandins, prostacyclins and thromboxan. Compering to steroids, their local use in ophthalmology is significantly smaller, although in many cases they appear to be very effective. The most numerous indications for their application is to inhibit myosis during cataract extraction with artificial lens implantation, cystoid macular oedema prophylaxis and treatment, painkilling following corneal refractive surgery, following foreign body removal. Adverse events are not frequent and include burning, redness, itching. They should not be used in patients with hypersensitivity to NSAIDs derivatives. There are few preparation available consisting of ketorolac, bromfenac, nepafenac, diklofenac and indometacin. Indications for their use are expanding, and also new preparations are characterized by good tolerance and safety which may influence their extended use.
