Summary: Dry eye syndrome is an eye disease caused by decreased tear production or increased tear film evaporation. The common cause of severe dry eye cases is Sjögren?s syndrome ? chronic autoimmune disorder characterized by infiltration of exocrine glands, particularly the salivary and lacrimal glands by lymphocytes and plasma cells. The classic signs of Sjögren?s syndrome include xerostomia and xerophthalmia related to diminished lacrimal and salivary gland function. Sjögren?s syndrome exists in a primary and secondary form. The basic treatment of dry eye syndrome consists in regular use of eye drops and gels without preservatives which help to stabilize the tear film and provide protection to cornea and conjuctiva. The aim of this paper is to present the influence of pilocarpine hydrochloride administered orally on attenuation the severe dry eye symptoms. Pilocarpine is a cholinergic parasympathomimetic agent exerting a broad spectrum of pharmacologic effects with predominant muscarinic action. Pilocarpine, in appropriate dosage, can increase secretion by the exocrine glands. The author also describes other methods of pharmacological and surgical therapies which play role in severe dry eye syndrome treatment.
